RN-led care team

Home health care from Cottage Home Care brings skilled nursing and clinical support directly to your door — so recovery, chronic condition management, and complex medical needs happen at home instead of a hospital or facility. Every plan begins with an in-home visit from a Registered Nurse who assesses your loved one’s health, medications, and daily goals, then coordinates licensed nurses and caregivers around a personalized plan of care. Services may include wound care, medication management, vital sign monitoring, post-surgical recovery, and coordination with physicians. Cottage’s RN-led team supervises every visit and adjusts the plan as conditions change. For eligible patients, Medicare and Medicaid often cover home health at little or no cost. Serving 6+ states with local clinical teams, Cottage Home Health keeps people safe, stable, and as independent as possible — with compassionate care that treats the whole person, not just the diagnosis.

What Is Home Health Care?

Your Care Starts With an RN Home Visit

Every Cottage Home Care client begins with an in-home visit from a Registered Nurse. The RN gets to know your loved one's daily routine, personal needs, and family goals, then builds a complete, personalized plan of care.

This plan guides our trained, compassionate caregivers as they provide hands-on support such as personal care, companionship, meal preparation, mobility assistance, medication reminders, and light housekeeping, helping your loved one stay safe, comfortable, and independent at home.

With Cottage Home Care, every family gets a plan built around what matters most, guided by an RN and delivered with heart.
