What Is Managed Long-Term Care (MLTC)?
MLTC is a New York Medicaid program. It pays for home care so you can stay at home instead of moving to a nursing home. A plan approved by the state sets up and manages your care.
Home-Based Support
Your aide comes to your home. You get help with daily tasks and stay where you are most comfortable.
Medicaid-Funded Services
Most Medicaid members pay nothing for covered home care. The plan handles the costs.
Personalized Care Coordination
A care manager is assigned to you. They set up your services and stay in touch when things change.

Who May Qualify for MLTC in New York?
The state decides who qualifies for MLTC. It is based on your Medicaid, your health, and how long you need care. We can help you check before you apply.
Not sure if you qualify? Call us. We will check your Medicaid for free. Also note: you cannot be in MLTC and HARP, Health Homes, or PACE at the same time.
- Active or qualifying New York Medicaid
- Meet the age or program rules for the plan type you want
- Need home care for more than 120 days
- Pass the health check through the New York Independent Assessor Program (NYIAP)
- Live in the service area of the plan you pick
- Can stay at home safely with the right help in place
You cannot be enrolled in MLTC while also enrolled in NHTD, an OPWDD waiver, or a Health Home, since these are separate care management structures.
If you don't currently qualify for MLTC, Private Pay home care may still be an option.
These are real families in your neighborhood who made the same call you're thinking about making.

How to Start MLTC Home Care
Cottage Home Care can guide you through each step, whether you are already enrolled or just getting started.
- 1
Confirm your Medicaid
Active New York Medicaid must be in place before the MLTC process can begin. If you're not enrolled yet, we can point you in the right direction.
- 2
Complete the NYIA Assessment
The New York Independent Assessor (NYIA) evaluates medical eligibility for long-term home care. This determines whether you qualify to move forward.
- 3
Select an MLTC plan
Once approved, you choose a Managed Long-Term Care plan. The plan's own RN then assesses your needs and finalizes your care hours.
- 4
Care starts
We match you with a certified aide and confirm your authorization is active before your first visit.
